Indiana Adoption Network

Indiana adoptee registry information can be found at IN.GOV and at the Indiana Adoption Netowrk.
Registry information- please know the state registry is free- you may register now- however you all will need to register again after July 1,2018.

Indiana Adoptee Registry Information Until July 2018

Until the records are opened on 7/1/2018, you must register with the Indiana State Department of Health Mutual Consent Registry to obtain information. Follow the directions below.
1.Go to the
2.In the search bar type in – adoption information- click on link-
3.Scroll to the bottom of the page
4.Click on link for Identifying information – form 47896
5.Click on link for Non-identifying information state form 47897
6.Fill out both forms with information you know
7. Copy driver’s license or other state issued photo ID.
8. Mail to address on bottom of 47896 and 47897 OR Scan and email both forms and copy of ID to –
In 4-6 weeks if you have not received confirmation from the state board of health contact – Darci Haight 317-233-7279.
